Two forms are necessary to be “cleared” by the Athletic Department.
They are (1) a Physical Form (this is good for two years), and (2) Insurance and Parent/Guardian Consent Form (one form with both of these on it; good for one year).
In-season, all students must have these on file with the athletic department in order to participate.
Unlike other schools in southern Oregon, Phoenix High School does not charge a “Pay-to-Participate” fee. The Phoenix-Talent School voted unanimously on August 8, 2019 to suspend "Pay-to-Participate" fees in hopes of increasing participating in extra-curricular and co-curricular activities at our schools. Students will still need to have proof of a sports physical, proof of insurance and a signed parent consent form to participate.
