Participation Requirements


Two forms are necessary to be “cleared” by the Athletic Department.
They are (1) a Physical Form (this is good for two years), and (2) Insurance and Parent/Guardian Consent Form (one form with both of these on it; good for one year).  
In-season, all students must have these on file with the athletic department in order to participate.
 

Like all schools in southern Oregon, Phoenix HS enforces a “Pay-to-Participate” fee. 
For 2015-2016, it is $75/sport ($35/sport if the student qualifies for free and reduced lunch) with a family cap of $200.  This family cap encompasses both middle and high school team participation.  If there is difficulty paying at the beginning of a season, please speak to the Athletic Director to set up a payment plan.

 

A Handbook for the Parents/Guardians of Student-Athletes
This material includes the athletic department philosophy; eligibility and participation regulations; extracurricular expectations; tips for productive communication between parents, coaches and the department; alcohol, tobacco and illegal drug policy; commentary on team selection, and; program level goals
